We let the opposing coach choose which player to remove (rather than it being random).
We do the same. We ask that players do it as soon as they notice, and that all players count (all the way to eleven!) their players before kick off to try to avoid the situation.
Certain unscrupulous individuals (ahem!) have been known to wait for someones gutter runner to charge down the pitch in to their half and then say "oh, 12 players, I'll have that gutter runner off, thanks for the ball" , so its better to check your team really.
