Am I correct in interpreting the rulebook in saying that Throws, Kicks, and Throw-ins are the only time a ball bounces out of an empty square?

In general yes.
If you push back and knock down a player in the square in which the ball lies and then sends him to either KO or D&I then it still bounces once.
And if you makes a hand off to an empty square it also bounces once.

Doesn't matter if you pass or not the armour... the ball bounces in every case, if you push somebody into the square where the ball lies. Wink

Yes. The rule of thumb is that if the ball traveled some distance, it will bounce.

Would that not depend on when you roll for scatter?

And technically the ball would scatter as soon as the player enters the square and not after they have hit the floor and been knocked out. Wink

Really? We play it
If a player is just pused into a square the ball will scatter after the blocking player decieds to follow up. No pick up attempt can be made by the player that was pushed into the ball, unless the ball bounces back to them.

If a player is knocked over onto the ball then blocking player decieds to follow up or not. Then roll for AV/Injury. Then the ball will scatter.

Well, the prone player has entered the square, so you should scatter the ball (and let it come to rest) before you roll AV/Inj.

Close, but not the same. If you do the AV/Inj before the scatter, the player could be removed from the field, and then the ball could scatter back to that square.

You roll scatter after Av/Injury. The ball is always scattered - but because that square may become empty it can effect where it ends up.

I think I've always played as Doubleskulls described. I've always assumed that the AV roll is a product of the block and so should happen before the scatter. I would play:

Pushback
Follow Up
Armour Roll
Scatter

I'm not so sure that could be backed up in the rules but it seems the most logical way to play to me.
